Not All There | O que significa esta expressão?

Às vezes uma expressão aparentemente simples pode ter um significado bem diferente, e é justamente esse o caso da expressão not all there. É fácil de ver que not all there significa “não inteiramente lá”, mas que sentido faz dizer que uma pessoa is not all there?
